 | Otto Modell Headstone. Status: Located; On September 25th 2015 we visited Evergreen Cemetery and Mausoleum in Evergreen Park Illinois. The Cemetery had changed management since Otto was buried and records for the older single grave section were very poor. There was no map available for that section and the markings were not very good. The cemetery staff was quite helpful and although it took a while eventually the grave site was located and a marker that was six inches below ground level was located. The Cemetery staff is going to raise the headstone.
